Thirty-eight individuals (mean age: 34.8 years old) participating in a 3-month yoga and meditation retreat were assessed before and after the intervention for psychometric measures, brain derived neurotrophic factor (BDNF), circadian salivary cortisol levels, and pro- and anti-inflammatory cytokines. Participation in the retreat was found to be associated with decreases in self-reported anxiety and depression as well as increases in mindfulness. As hypothesized, increases in the plasma levels of BDNF and increases in the magnitude of the cortisol awakening response (CAR) (...) were also observed. The normalized change in BDNF levels was inversely correlated with BSI-18 anxiety scores at both the pre-retreat (r = 0.40, p < 0.05) and post-retreat (r = 0.52, p < 0.005) such that those with greater anxiety scores tended to exhibit smaller pre- to post-retreat increases in plasma BDNF levels. In line with a hypothesized decrease in inflammatory processes resulting from the yoga and meditation practices, we found that the plasma level of the anti-inflammatory cytokine Interleukin-10 was increased and the pro-inflammatory cytokine Interleukin-12 was reduced after the retreat. Contrary to our initial hypotheses, plasma levels of other pro-inflammatory cytokines, including Interferon Gamma (IFN-γ), Tumor Necrosis Factor (TNF-α), Interleukin-1β (IL-1β), Interleukin-6 (IL-6), and Interleukin-8 (IL-8) were increased after the retreat. Given evidence from previous studies of the positive effects of meditative practices on mental fitness, autonomic homeostasis and inflammatory status, we hypothesize that these findings are related to the meditative practices throughout the retreat; however, some of the observed changes may also be related to other aspects of the retreat such as physical exercise-related components of the yoga practice and diet. We hypothesize that the patterns of change observed here reflect mind-body integration and well-being. The increased BDNF levels observed is a potential mediator between meditative practices and brain health, the increased CAR is likely a reflection of increased dynamic physiological arousal, and the relationship of the dual enhancement of pro- and anti-inflammatory cytokine changes to healthy immunologic functioning is discussed. Property rights are important institutions that influence economic performance and reflect the historical, cultural, and political realities of particular societies. Drawing on a variety of concepts from legal and economic studies, a framework for explaining the origin and evolution of property rights is developed and applied to the specific case of changing ground water rights in Nebraska. The Nebraska case is an interesting example of reliance on local control in regulating water use. Despite the importance of local initiatives in ground (...) water management, this case also illustrates the need for external support from the judicial and legislative systems. The evolution of ground water property rights in Nebraska, as in other parts of the United States, has been conditioned by historical circumstances and changing values and attitudes as well as by economic and political forces. In this paper I draw the distinction between intuitive and theory-relative accounts of the time reversal symmetry and identify problems with each. I then propose an alternative to these two types of accounts that steers a middle course between them and minimizes each account’s problems. This new account of time reversal requires that, when dealing with sets of physical theories that satisfy certain constraints, we determine all of the discrete symmetries of the physical laws we are interested in and look (...) for involutions that leave spatial coordinates unaffected and that act consistently across our physical laws. This new account of time reversal has the interesting feature that it makes the nature of the time reversal symmetry an empirical feature of the world without requiring us to assume that any particular physical theory is time reversal invariant from the start. Finally, I provide an analysis of several toy cases that reveals differences between my new account of time reversal and its competitors. (shrink)

Coastal ecosystems are increasingly dominated by humans. Consequently, the human dimensions of sustainability science have become an integral part of emerging coastal governance and management practices. But if we are to avoid the harsh lessons of land management, coastal decision makers must recognize that humans are one of the more coastally dependent species in the biosphere. RationaleSurrogates of critically ill patients often have inaccurate expectations about prognosis. Yet there is little research on how intensive care unit clinicians should discuss prognosis, and existing expert opinion-based recommendations give only general guidance that has not been validated with surrogate decision makers.ObjectiveTo determine the perspectives of key stakeholders regarding how prognostic information should be conveyed in critical illness.MethodsThis was a multicenter study at three academic medical centers in California, Pennsylvania, and Washington. One hundred eighteen key stakeholders completed in-depth semistructured (...) interviews. Participants included 47 surrogates of adult patients with acute respiratory distress syndrome; 45 clinicians working in study ICUs, including physicians, nurses, social workers, and spiritual care providers; and 26 experts in health communication, decision science, ethics, family-centered care, geriatrics, healthcare disparities, palliative care, psychology, psychiatry, and critical care.Measurements and main resultsThere was broad support among surrogates for existing expert recommendations, including truthful prognostic disclosure, emotional support, tailoring the disclosure strategy to each family's needs, and checking for understanding. In addition, stakeholders offered suggestions that add specificity to existing recommendations, including: In addition to conveying prognostic estimates, clinicians should help families "see the prognosis for themselves" by showing families radiographic images and explaining the clinical significance of physical manifestations of severe disease at the bedside. Many physicians did not support using numeric estimates to convey prognosis to families, whereas many surrogates, clinicians from other disciplines, and experts believed numbers could be helpful. Clinicians should conceptualize prognostic communication as an iterative process that begins with a preliminary mention of the possibility of death early in the ICU stay and becomes more detailed as the clinical situation develops. Although prognostic information should be initially disclosed by physicians, other members of the multidisciplinary team-nurses, social workers, and spiritual care providers-should be given explicit role responsibilities to reinforce physicians' prognostications and help families process a poor prognosis emotionally.ConclusionsFamily members, clinicians, and experts identified specific communication behaviors that clinicians should use to discuss prognosis in the critical care setting. These findings extend existing opinion-based recommendations and should guide interventions to improve communication about prognosis in ICUs. (shrink)

We argue that the simplicity condition on a probability function on sentences of a predicate language L that it takes only finitely many values on the sentences of any finite sublanguage of L can be viewed as rational. We then go on to investigate consequences of this condition, linking it to the model theoretic notion of quantifier elimination.

Le débat entre E. Peterson et C. Schmitt des années 1930 sur le théologico-politique a laissé une question irrésolue qui resurgit dans les sociétés pluralistes postmodernes. La nouvelle théologie politique du théologien de Münster J.B. Metz la réactive en proposant au fil de son oeuvre une éthique politique comme éthique du changement social. En sous-estimant le caractère polémique du politique et la fragilité des sujets dont l’identité est devenue précaire, Metz parvient sûrement à une théologie critique mais demeure incertain (...) sur le versant de l’invention politique proprement dite de la société. L’opposition de J. Milbank à cette théologie politique ne dirime pas la question et demeure dans un geste uniquement critique en proposant une contre-éthique, une contre-ontologie, un contre-royaume par la mise en oeuvre de pratiques narratives, rituelles et sociales. Un débat entre la théologie de J.B. Metz et la pensée communautariste de S. Hauerwas pourrait peut-être affronter la question de l’avènement des sujets capables d’invention politique dans les sociétés complexes de la postmodernité. (shrink)

In Pure Inductive Logic, the rational principle of Predicate Exchangeability states that permuting the predicates in a given language L and replacing each occurrence of a predicate in an L-sentence phi according to this permutation should not change our belief in the truth of phi. In this paper we study when a prior probability function w on a purely unary language L satisfying Predicate Exchangeability also satisfies the principle of Unary Language Invariance.

Little is in print on the development in Britain of university disciplines including the academic study of education. This paper provides a historical narrative of the study of adolescence from the 1870 Education Act to the contemporary day. Adolescence textbooks are examined in the context of changes in the education system, such as the demand for secondary education and the expansion of teacher education. Changing emphases in adolescence research are traced through various publications to illustrate curriculum developments. The important contributions (...) of Dame Olive Wheeler, Professor W. D. Wall and other leading writers on adolescence are evaluated. It is clear that in over a century of national educational provision British teachers have never faced a great choice of adolescence texts and the paper concludes with some reflections on this observation. On cherche à montrer par l’analyse des textes que l’interprétation du maître ès arts s’écarte radicalement de la pensée du Cordouan qu’il prétend suivre. Tandis qu’Averroès suppose l’existence continue d’au moins un philosophe dans le monde, Jean de (...) Jandun, rejetant comme irréaliste l’idée d’un homme parfait en qui s’incarnerait tout le savoir, et tributaire d’une erreur de traduction arabo-latine qui donne « sujet » pour « lieu », soutient qu’il n’y a chaque fois de perfection que si l’on totalise tous les savoirs particuliers des différents penseurs en activité sur la terre. Autrement dit, la philosophie est parfaite, certes, mais il n’y a pas de philosophe parfait. C’est coniunctim que la science est achevée, et non pas divisim. Un tel constat, qui rapproche l’analyse de Jean de celle menée par Dante au début de sa Monarchia, permet deux révisions : d’une part, l’idée d’un « averroïsme » latin réactionnaire, se contentant de répéter les paroles du maître, est à revoir ; d’autre part, cela signifie qu’il n’y a pas chez Jean les bases théoriques de l’idéal éthico-intellectualiste dont l’historiographie moderne assure qu’il en fut le champion parisien. How does talk work, and can we engage the public in a dialogue about the scientific study of talk? This article presents a history, critical evaluation and empirical illustration of the public science of talk. We chart the public ethos of conversation analysis that treats talk as an inherently public phenomenon and its transcribed recordings as public data. We examine the inherent contradictions that conversation analysis is simultaneously obscure yet highly cited; it studies an object that people understand intuitively, yet (...) routinely produces counter-intuitive findings about talk. We describe a novel methodology for engaging the public in a science exhibition event and show how our ‘conversational rollercoaster’ used live recording, transcription and public-led analysis to address the challenge of demonstrating how talk can become an informative object of scientific research. We conclude by encouraging researchers not only to engage in a public dialogue but also to find ways to actively engage people in taking a scientific approach to talk as a pervasive, structural feature of their everyday lives. The Aristotelian‐Thomist philosopher holds that human intellectual knowledge is possible because of the order in the world and natural human capacities. It is the position of this paper that there is a shared human form or nature that unites all humanity as members of the same kind. Moral treatment is due to every human being because they are human, and is not based upon expression of abilities. Humans have substantial dynamic existence in the world, an existence which overflows in expressive (...) relationships. As both patient and health professional are human, human nature forms the natural foundation of health care. This paper looks towards human nature for moral guidance. The therapeutic relationship is seen as a part of the interpersonal moral space formed by human relationality, which tends towards community – in this case, the healthcare system. The therapeutic relationship is also a source of moral responsibility, as illness makes the patient vulnerable, while knowledge and nursing capacities generate in the nurse a duty to care. Nursing theory serves to connect philosophical reflection and nursing practice. Imogene King's conceptual system and theory of Goal Attainment is the theory that follows from the perspective of human person being presented. This synthesis of philosophy and theory is developed with the goal of shedding light on healthcare decisions in palliative care. The article concludes with the acknowledgement that the complexity of contextualized individual decisions requires the insight and discipline of the moral practitioner, and provides some thoughts on how education, development, and refinement transform an individual into a nurse.
